Registered Nurse (RN)

Comfortlink Healthcare is seeking experienced, compassionate, and dependable Registered Nurses to join our clinical team. Our RNs play an important role in client assessments, care planning, skilled nursing services, clinical oversight, and ongoing support for clients and caregivers.

Responsibilities

Complete initial and ongoing nursing assessments
Develop and update individualized care plans
Provide skilled nursing care and client education
Monitor client health status and report significant changes
Complete supervisory visits and reassessments as assigned
Provide clinical oversight and support to LPNs and caregivers
Communicate with physicians, families, case managers, and other members of the care team
Maintain accurate and timely clinical documentation
Ensure nursing services are provided in accordance with applicable regulations and company policies

Requirements

Current and unrestricted Georgia RN license
CPR/First Aid certification
Reliable transportation
Strong assessment, communication, and documentation skills
Ability to work independently in community-based settings
Compassionate, organized, and dependable
Home health, private home care, long-term care, or community nursing experience preferred

Responsibilities

Provide direct nursing care within the LPN scope of practice

Caregiver / CNA

Comfortlink Healthcare is seeking compassionate, dependable, and professional Caregivers and Certified Nursing Assistants (CNAs) to provide quality care and support to individuals in their homes and community settings.

Responsibilities

Assist clients with activities of daily living, including bathing, grooming, dressing, toileting, and personal hygiene
Assist with transfers, positioning, mobility, and ambulation as needed
Prepare meals and assist with feeding when required
Provide medication reminders within the caregiver’s permitted scope of practice
Perform light housekeeping, laundry, and other tasks necessary to maintain a clean and safe environment
Provide companionship and emotional support
Assist clients with community activities, errands, and appointments when assigned
Observe and promptly report changes in the client’s condition to the appropriate supervisor
Follow the client’s individualized plan of care
Complete required documentation accurately and on time
Clock in and out according to company procedures
Immediately report incidents, injuries, hospitalizations, emergencies, or concerns
Maintain client confidentiality and comply with HIPAA requirements
Follow Comfortlink Healthcare policies, procedures, and professional standards

Requirements

CNA certification or previous caregiving/personal care experience preferred
CPR/First Aid certification or willingness to obtain required certification
Must successfully complete required background screening and employment requirements
Reliable transportation and dependable attendance
Ability to communicate effectively with clients, families, and staff
Compassionate, patient, respectful, and professional
Ability to safely perform the physical duties required to assist clients
